MINOR (CHILD) TRAVEL CONSENT FORM

Please fill in the necessary details to generate a Travel Consent For Minor. After submitting the information, you will receive the documents for your review. The fee for this service is $40.00 + Office Visit | Notarization = $70.00 if you visit the office. $75.00 for online service and notarization (Pre-payment is required for the form to be completed) and $105 plus mobile notary services authorization. (Pre-payment is required for the form to be completed) .The completed document will be sent in PDF format with your provided information. This is the example of the form below.
